  • THe U.S. Department of Education has selected your file for review due to your unusual enrollment history.  This is defined by having attended multiple colleges/universities during the period of: 2021-2022, 2022-2023, 2023-2024, 2024-2025.  This process requires a review to determine whether there are valid reasons for your unusual enrollment and that you are earning credit and successfully completing your classes.  To be considered for financial aid you must complete and submit information about your enrollment records for each institution attended. (Note: In the process of reviewing your enrollment history, the Financial Aid Office will check the National Student Loan Data System (NSLDS) for a complete history of the grants and loans you have received.)

    a. Unless Admissions already has a copy on file, provide transcript copies from each college or university you have attended during the years listed above.

    b. Please provide documentation to support the explanation given.

    c. Incomplete UEH forms will not be provided to the committee.
